Seneca Middle School along with the help of Howd Medical Service will be offering low cost and convenient sports physicals this year!
- Thursday May 23rd 2019
- Open for students currently in 6th & 7th grades
- Physicals will take place at Seneca Middle School beginning at 8:30am
- The IESA history portion must be completed, with a parent signature, along with cash or check for $25.00 payable to Howd Medical LLC.
- The signed IESA form and money are due to the school nurse by Friday, May 17th.
- Sports Physicals are valid for 395 days after the date completed.
